Claim Processing Requirements

FFP - NATURAL DEATH 
 

1. Policy Document
    
2. Death Certificate (CERTIFIED COPY OF ENTRY IN REGISTRY OF DEATH), Post-mortem, Medical certificate of cause of death.
    
3. Completed Claim forms (Available at ELAC)
    
4. Photo Id of Claimant (Voter’s ID, Passport, Driver’s License etc)
    
5. Photo ID of Beneficiary if 18yrs and above (on death of policyholder).
    
6. Photo ID of Trustee (on death of policyholder) if  Beneficiary is a minor
   
    

FFP - ACCIDENTAL DEATH

1.  Policy Document
    
2.  Death Certificate or Doctor’s report of cause of death
    
3.  Police Report, Medical Report
    
4.  Completed Claim forms (Available at ELAC)
    
5.  Photo Id of Claimant (Voter’s ID, Passport, Driver’s License etc)
